Omen of amelioration doesn’t work randomly

Ps5 user - over the last week I’ve repeatedly observed a bug where when I die with Omen of Amelioration active, I will instead lose XP as if it wasn’t active and the still active omen will remain in my inventory. If I fully close out of the game and restart, it will begin to work correctly again. This also appears to coincide with another bug where maps I’m currently in will regain their fog of war if I leave to unload loot and return. So they have have the same root cause.

Immaterial to the issue

I also had a bug like this yesterday, I think it is because I had multiple active instead of just one, since I just enabled 1 in my inventory it hasn’t happened since.
