Uniwue map cleared but no passive achuevement granted

i have completed the sikent cave uniwue map at least twice - killed the boss and no uodate or passive reward.

Last night, looked the map node. it said complete with a lvl 11 map. i dout it a rare, 4 affix map, killed all 4 support essences and theb went on to kill the main essence map boss in the centre of the map. got the lott drops of greater essences but the map did not update saying I completed the map objective. I took xbox screen shits showing zero monsters left in the map with the loot and object not cleared.

I had this happen on the untainted paradice map as well.
