Breachstone glitch and lost breachstone.

I experienced a bug in a breachstone map after entering. The breach hand was missing and there was a purple crater where it was supposed to be. The hand showed up on the mini map in the corner but not in the map itself. The map was also empty with no monsters due to the inability to breach. I have screenshots if needed on my Xbox account. I’m kindly requesting my breachstone to be refunded. Thank you for your time and assistance.
