Lost waystone and cannot activate tower in endgame on controller

Scenario:

1) In endgame, on an obelisk tower, there's one waystone slotted in (let's call it Waystone A).
2) On controller, with the inventory open, navigate to a waystone on your inventory (let's call this one Waystone B) and press 'X' button

Expected the waystone A to swap with Waystone B

What happened was that waystone B did go into the tower's slot, but waystone A disappeared. Waystone A is not in my inventory, and it's not in the tower slot. I also cannot activate the tower anymore. Please help.

Update 2: I found my missing waystone A on an adjacent node NEXT to the tower for some reason... what the hell, how did the waystone end up there? lol
