Minion bugs during weapon swaps

I haven't been able to isolate this, but I've noticed both these in several maps.

When weapon swapping, sometimes the minions do not respawn at all. I'm left with reserved spirit but nothing to show for it until I reset my gem settings or teleport to my hideout.

Other times when weapon swapping the minions unsummoned die. I think this happens if a remaining minion dies during this period.
