Omen of Amelioration not working

I had an active Omen of Amelioration and an active Omen of Resurgence in my inventory. The latter activated prior to my character's death but the former didn't.

If it makes a difference, I had multiple Omens of Ameliorations in my inventory, but only one was active.

Please let me know if I'm missing something! Thank you.

Only one Omen can be activated per combat area.
