Gemcrust Skull - Azak Bog Cruel Difficulty Missing Spirit

Upon completing the campaigne and reaching t15 maps I have noticed I am missing 40 spirits - the exact amount the Azak Bog Witch Drops in Cruel Act 3.

The Boss Icon with the + shows as not completed on my map.
I have killed the witch several times now, unfortunately no luck.

I am not sure if it's an issue with the item not droping, or the spirit gain not registering.

Things I have tried:
- Killing the Witch on Cruel (at least 6 times)
- Killing all Spirit bosses and then the Witch on Cruel
- Entering and re-entering the instance
- Talking to Servi about Local Knowledge and then killing the boss
- Entering the instance with someone who has not completed the map before (they received the item, I did not).
- I have checked all my stash tabs if the item is not stored in one of them
- Killing the boss, force shutting the game and logging back in
- Cleaing the whole map of all monsters and completing the other event before killing the Witch

Unfortunately, I have invested already a lot of time and currency into my character and I wanted to continue progressing on it, so making a new character is currently out of question, as I simply won't have the time to go through the campaign again (possibly risking the bug happening again too).

Does anyone have a possible solution to the issue or is any fix for this planned in the near future?

Additional information

I believe it could be caused by the item dropping, and the the server crashing and rolling back a bit (like it did during first few days of the League). And with the item not being used immedaitely after pick up, and disappearing after the rollback the game might think I have already had it dropped, thus not dropping it again.

I have noticed many people in chat talking about this issue.
Weirdest thing is that I, even though I have finished the quest and gained the spirit from the item, still have the Azag Bog 'boss' icon highlighted, when it should be shown X-ed out.
There seems to be an ongoing huge issue with items that grand bonuses overall and hope they would change it to NPCs awarding the player with it instead (just like Doriyani does when you do not pick up the Atlas skillbook from corruption boss in maps).

With the new update that dropped my Inganuk icon is now crossed out, however I am still missing 40 spirit from her.
Tried killing her again - nothing.

I have 60 without equipment
162 with equipment

With King of the Mist in Act 1 giving 30
Ingabuk in Act 3 giving 30
And then Ingabuk in Act 3 Cruel giving 40

So I am missing the 40

is this bug fixed @GGG?
