Kill the forge master at Molten Forge on Cruel
|

Renlys quest gives salvage bench and the quest in Act 3 for the Molten Vaults gives the Reforge - I believe you talk to the NPC by the Jungle Ruins entrance I forgot his name. Hope that helps.
