Chaos Orbs

Is there supposed to be a chance of a Chaos Orb either not removing then adding an affix OR removing and then re-adding the same affix (at the same value)? Because both chaos orbs I've used since the 0.2 have resulted in no changes to the item.

I've used them on an Emerald and a Buckler.
