Disappearing Essence Pack

I was leveling and running a Warrior through the campaign. While in Act 2: Valley of the Titans I encountered a Essence pack. I think it was a Essence of Body. As far as I can tell, it was one of the large lumbering Skeleton monsters. When I activated the pack, all of the monsters in the pack did not spawn and they just disappeared.

This also happened to me in Valley of the Titans and Titan's Grotto

Just happened to me in Deshar (Act 2, Cruel). None of the monsters appeared upon breaking the essence, but there does appear to be corpses on the ground corresponding to them.
