Breachstone (Twisted Domain)

Upon respawning after a death in the "Twisted Domain" no hand was visible from the ground and closing the game or logging in and out or teleporting back and forth changed anything. I have since moved on as I knew I had more than likely lost my breachstone chance but future fixes would be nice. I have a screenshot but can't seem to attach it to the thread.
