Omen of Resurgence

Lost my 82 infernalist a few days ago despite the omen being activated in my inventory. I died without in being activated.

As for the omen not proccing, the wording is very tricky. It only activates upon reaching low life. IIRC, LL is 35% life by default. If you're at 36% or higher life and take a single hit that exceeds your current life, you never reach low life status, so the omen never procs. Even if you're at 100% life and take a single hit that exceeds 100% life, you still never reach low life status.

Thanks for your input. I thought of something like that, but I had 16-18k ES and 2-3k life. No boss or anything, cleairng easily the area. So I accept the idea, make sense but it's an hell of a shot isn't it?

Unfortunately I have no visibility on what your gear was like or how rippy the map mods were. But if ES was your only layer of eHP (and not running anything like ES bypass), then a rare enemy with a lot of HP and corpse explosion I can easily see going through that in 1 hit.
