Ritual Altar cannot be activated

I was running a map with the destructive play minor idol (spawns bosses to accompany map boss when under maven)

Ritual Altar which is located in 'boss area' of a map cannot be activated after everyone are killed.

I think that one or two bosses might be killed out of Altar.
One of the bosses was 'Tukohama', his totems were inside of altar area, but he himself was killed out of altar area.
