Mechanic-blocking unique Idols not blocking the mechanic

I have a unique anti-Ultimatum idol socketed, and I still found an Ultimatum in a map. I dropped an inscribed Ultimatum in the same map, not sure where it came from or how I got it.

Wasn't in a Kirac Mission, other idols are applying 18% increased chance for Ultimatum.

Same problem im dealing with too, used unique idol to block breach but it still appear in map.
There’s no “chance to contain breach” on other idol im using
