Broken Citadels

I know these topics have been posted 100 times, but why haven't they been fixed yet? I continue to clear these, get the fragments, and still Fail the instance. This is becoming a problem since some of the areas they are in, completely block the path for me to continue on without finding super annoying ways around it. If I fail it, that's on me, but if I clear it, I shouldn't be punished. Please, someone fix this issue.
