Ignagduk, The Bog Witch - Passive Points

Recently I went back to playing my Monk, and realized that I am missing 4 passive points from the campaign (which I have had prior to this). Easily got back 2 of them, but the witch is not dropping the tome and Skull on campaign map is not crossed out no matter how many times i kill her, with ritual and without ritual.

Ignagduk does not give passive points. In Act 3 Ignagduk gives 30 spirit, in Act 6 40 spirit. The uncompleted icon in Act 6 is just a visual bug.
