Omen of resurgence doesn't activate second time in a map

I used one Omen of resurgence in the map activated another but it didnt work respawned with it being activated

Only one non-crafting omen may be triggered and consumed in any single zone/instance. You are not able to chain their use by design.

There should be text about only one omen being consumed during combat on the item (in the white text at the bottom, I believe).
