Can anyone help explain this please?

I've been playing PoE since the closed beta more than a decade ago and I've never seen anything like it, and do not understand it.

Context: This season, I had decided to try Ruthless SSF for the first time. While much of it was decent, I was struggling sustaining red maps. For the first and only time, a t16 map dropped for me (with a long gap between t11 and t16), and I had been saving it until I was sure I had both the currency to roll it properly, the gear and skill to complete it, and the scarabs to complement it. Having finally completed these tasks, I prepared by putting a few weapons in my inventory (for animated weapon, just in case I got ambushed by something near the opening of the map), selected the map, put it in the device with the scarabs and entered it.

Issue: I was able to play for approx 45 seconds or perhaps at most a minute (and had already found and picked up another t16 map!) when the client crashed to login with the error: unable to connect to game host.
I was not too worried, since I still had all portals open.
But when relogging in -- which went smoothly, no portals were visible. None.
My inventory showed a full selection of the weapons I had prepared -- but no map or scarabs.
How can this be? If for whatever reason the game crashed, shouldn't it have saved? And if so, it must have saved AFTER the time I had opened the map (since it was missing). BUT if it saved AFTER I had opened the map, then why were there zero portals?? If it saved BEFORE I opened the map, thus explaining the lack of portals, why is the map gone?
Despite -- for the first time in over a decade -- quitting PoE now (who knows, perhaps for some time) out of frustration, I would like some form of clarification.

And this crash happened after I was playing for a minute, but was saved to a point immediately after the instance was created?
