insta dying to mobs when i use abilities

im instantly dying to to random mobs sometimes when im in maps , i check to see if it was reflected damage back to me but there was no such mod and i used molten shell and still problem persisted, ive only been playing for a week so i dont know about any factors that could be at play , but it seems like a bug to me.. debilitating especially if i have alot of mods and scarabs on the rotation
