Lost loot in HC

I am on console (PS5), all of my loot disapears after a few seconds of it being on the ground. I cannot see chests or gold when entering treasure rooms. The items I have seen such as uncut gems, exalted, jeweler orbs and gear have all disappeared before I could get to them.
