Full energy shield with chaos Inoculation and just die

With full energy shield and chaos Inoculation active a boss casts fire and I just die. No damage to the energy shield completely bypassing with chaos Inoculation active. Happened 2 times. No idea what is doing the killing.

I do not have the hat. I found out what killed me it was the mana pot that uses 15% of life. use 4 times and I die. So new pot.
