PoE2 Monk - What am I doing wrong ?

Having trouble clearing Breach and Delirium maps - so just need a general direction - I've been trying to improve stuff but feeling kinda lost right now - is it just the passives or does my gear suck ? I'm still trying things out myself and haven't followed any builds online.
