Deferred item not showing up anymore. Don't know if this is a bug?

I had Omen of Whittling in one of the Ritual rewards. I deferred an item, yet after 10+ ritual maps with rerolling on each I haven't seen it again.
I know it is not 100% chance to have it on the next ritual. But 10+ runs + rerolls (20 total) really looks like a bug. Thing to mention is that on one of ritual maps i died without checking the rewards. I wonder if it could be the reason for deferred item to get vanished since I haven't deferred it again. Has anyone had that as well? PoE nowadays looks more and more like a fork server with tons of custom weird experimental stuff rather than original game with carefully thought out balance brough live to players. I can confirm if you deferred it and it showed up again on a map you died on without deferring it again it will poof.

Well that is just sad. It wasn't like that on PoE1.
PoE nowadays looks more and more like a fork server with tons of custom weird experimental stuff rather than original game with carefully thought out balance brough live to players.
