What about a simple in-built system:
If player recieves damage from a single hit larger than their current EHP (Life + ES + Mana if MoM) their Life is reduced to 1 but they are not killed instantly. Instant death occurs only when damage recieved is at least 2x higher than player's EHP. Damage is calculated ater all reductions.
This could remove a large number of sudden deaths and ES will be still the best defense against one-shots (as intended). Also XP death penalty and one portal per map will be much more bearable for many players.

Nah. What you're asking for is interactive gameplay and the poe vets do not like that.
This game is all about holding a button down zooming thru maps while trying not to get too much drool on yourself. Sometimes, the game says "poof ur dead" and thats how the remaining playerbase likes it.

Not a bad idea.
Could be implemented as a Support Gem.
With some moderate to high spirit and stats requirements.
Meaning that to gain this advantage would be a trade off.
It would be a good application for the spirit of a Warrior class.
Heck could even make it a Spirit Gem on it's own and add Supports to it.
