Headhunter Heavy Belt Question

Ok, so there's 1 modifier from rare monsters that keeps getting me killed and I am curious if there is a way to control it, or make it stop being so random! It is the random teleporting. Am I clicking something that causes it to TP to a group or is it just random? And is there a way to stop it from doing this? Had a few deaths in breach due to this.
