Dying with Mind Over Matter passive + Chaos Inoculation

After the patch I have been dying with Dying with Mind Over Matter passive + Chaos Inoculation passives enabled, my mana pool is full but instantly die when my shield is gone. Is anyone else having this issue?

I found my problem it was my flask that killed me I changed it.

EDIT: After some tests, realised Mind Over Matter works like this

@Sephirya, yes it should work like that, but sometimes (not all the time) I die exactly how it looks like in your screenshot, still have mana in the mana pool, but I am dead.

@ugurhanb, maybe your on to something, my mana flask does take away 15% from life on use. That's probably it!
