Ancient Vows

I have a problem with finding the second artifact, I've already done act 2 for the second time, and I can't get it, maybe the problem is that when the first one dropped for me, it dropped in two copies at the same time from mobs that I killed at the same time, but after picking it up I only had one in my backpack.
This is my third character, I didn't have this problem before.
