The Disappearing 'Emperor'

I cached "Emperor" two days ago, and I am absolutely certain about it. My account is solely for my own use. However, I've hunted for 50 sacrificial maps and haven't encountered it or any new "Emperor" since. I refresh my items twice every time, and I don't think this is just a matter of luck anymore. Could you please provide an explanation?
