[Couch Coop] Map instance fade to black when one player dies

1. When mapping, after one player dies, the game fades to black after a while, sometimes even crashes, sometimes certain UI elements (eg. the skill bar) remain. Not immediately, after some random, varying time. After restart, we cannot reenter the map to pick up loot because of the death.

2. Also, player 2 cannot pick up maps dropped by a map boss if player 1 (who opened the map) died, even if shared loot is enabled.

Only workaround for both is to git gud.

It's a 99.99% guaranteed game crash if 1 player dies in couch coop and the other keeps playing, at least on PS5.

The only time it doesn't crash is if you manage to finish the map really fast after the death, and gate out.

I've seen it crash almost instantly after one player is dead, and I've seen it crash minutes later.

It makes mapping for couch coop players almost impossible, especially with all the instant death mechanics in the game. You constantly lose progress and map waystones due to the game crashing.

I've send in over 20 crash reports in the last couple a weeks.
So... maybe start looking at those? Maybe there is something in there that can help figure out whats going wrong.

Or even better... start testing couch coop, because there are tons of unforgivable stupid bugs in there, that you can't miss if you even tried the mode once.
