Can't tell if I got one shot or not, but definitely didnt trigger as its still in my inventory and I died.

Was it activated ? Its not enough to just have it in your inventory. You have to right click it there to activate.

i had two active omens of resurgance in my titan inventory expansion and i died...

bump, still an issue, died several times with the s**t being active in my inventory (only one resurgance omen, not more)
