Can not do "Ominous Altars"

I cleared the 4 Ritual Altars in Freythorn.
The boss did not spawn. The quest still sayxs "Search for Ritual Altars and cleanse them". HCSSF is the only way. Last bumped on Jan 23, 2026, 1:27:52 PM

Just happened to me as well, right now. Cleansed all Altars, every single mob, yet no boss spawned at all, and still "Search for Ritual Altars and cleanse them".
It's been a year since this glitch/bug and still not fixed? -.- |

Are you activating rituals or just killing mobs around them?
Sorry for the dumb question, never heard of this bug. |
