Mobs that spawn damaging objects and zones seem to do so per hit taken

One rare mod for example is "Lightning mirages when hit" - which has a melee character spawn half a dozen mirages in a second as they attack it.

It may be an issue that it triggers "no more than every x hits", which is not really a limiter with high attack speed builds.

Unsure if bug or feature/balance issue.

I'll add one more: the "periodic X explosions" feel like they may be bugged in a similar manner, where the rares disgorge half a dozen zones in an instant when my monk runs them through - are they coded so "periodically" ties to hp thresholds? So a melee character bursts them down and immediately finds itself surrounded by 6+ soon to explode zones?
