Missing Inscribed Ultimatum after Cruel acts start (Bug Report ID: #2742337722)

Scenario:
Going through "normal" acts - act 2 Chimeral Wetlands Chimera is killed and Inscribed Ultimatum dropeed.
Inscribed Ultimatum is used in Temple of Chaos to start chaos trilas. Trial failed Inscribed Ultimatum is taken back from the altair and kept in inventory.
Game progressing.
Moving to Cruel acts without completing Chaos Trial quest.
Inscribed Ultimatum still in inventory.
Inscribed Ultimatum dropped from invetory to the ground and MISSING.
Return to normal act 2 and killing Chimera again does not re-drop the Inscribed Ultimatum to complete the chaos trial quest.

Current state:
No possibility to use Inscribed Ultimatum for completing the quest.
Some workaround is to try to use different ultimatum to start the trial (unfortunately they are not so common drop) but they cannot be re-used as Inscribed Ultimatum is.

Expected state:
Way to retrieve the Inscribed Ultimatum to complete the quest in further game state.
