4th ascendency issues (both Trialmaster's Tower and Trial of Sekhemas)

On a previous run I put a fragment in before realizing there is no stash here, so I can't grab the other 2 fragments (already annoyed). On this run, I can't interact with the door, maybe because my fragment is still in there?

My 4th ascendency is completely blocked. I can't get into the towermaster's trial now and my trail of sekhemas 4th floor disconnects me with the terrain generator out of sync error every time I try to enter the instance.
