Consistent crash at boss fights each time with rollback

Any boss we do, it seems to crash.

The ID is from 3rd "restart", so that session has nothing in it , but the 2 before are where I consistently crash when I weapon swap next to the boss. It does not seem to trigger next to rares or other monsters, just bosses. I am not sure what triggers it, but the moment my minions pop, the instance is gone along with maps, loot and all inventory content.
