Game not using GPU correctly?

It's completely random when this happens but sometimes I'll boot the game up and I'll get ~40 fps which is far below my expected performance.

I've been clean reinstalling my drivers and then rebooting which seems to sometimes resolve the issue and I get a rock solid ~100 fps.

Computer specs: Intel 14700k, Nvidia RTX 3090 FE, 24gb ram, Asus ROG STRIX Z790-I motherboard



I've also looked at this reddit thread (https://www.reddit.com/r/PathOfExile2/comments/1h8g9jk/if_youre_on_steam_and_experiencing_very_low_fps/) and tried every single possible solution proposed to resolve this issue and nothing has worked permanently. I suspect that the game is trying to use my internal Intel graphics but I have disabled it in the BIOS and in-game it says it's using my 3090. One of the top comments in the thread linked is to re-slot your GPU into a different PCI slot but my motherboard only has one PCI slot. I've also tried Vulkan and DX11 which both give similar abysmal performance. I also use the Nvidia app for my drivers and the app recognizes Path of Exile 2 properly. I do not use the Nvidia app to "optimize" graphics settings.

When I manage to get the game running properly (completely by chance with no discernable pattern on how to fix), my graphics graph has every single line essentially be a solid horizontal line.
