Instances persistence across different parties

Joining instances and they are persistent across various groups the patches are leading to lag do to not terminating instances unvisited by players, sometimes for hours or days. In addition my level 86 when assisting on a Act 1 boss is have difficulty killing simple bosses and mobs on the map now. Something is not right.
