2 skills points missing

With one of my characters I have missing 2 skills point, I'm sure did every side quest regarless the reward and I cant find which one didnt drop the skill book reward.
I have every marker in the map greyed out meaning that I killed/finished that objective, except for the camps, rootdredge and ignagduk (both act 3) in cruel, I killed them a couple of times to be sure but they not get greyed out.
