Map Crashes and Everything gets Rolled Back

I have experienced several instance crashes while mapping and both loot and experience were rolled back as if the map had never been run. Every time this has happened, it was while doing a breach in delirium. I'm not sure if those have any effect on them. Also, there have been many crashes when running a breach.
