Second ascension bug

Upon defeating the chimera giving me the invitation to my second ascendency through ultimatum, my inventory was full. Picking up the item put me on the quest for the ultimatum, the green item disappeared, and was nowhere in my inventory. I could go to the temple but couldn't start the trials. The trialmaster was not here to give me a new one and killing the chimera boss again for it to drop again didn't work either. I had to find the next invitation from after the gateway I'm assuming the bug happens with sanctum as well but since the trialmaster is already next to the keyhole maybe not
