Missing 2 Passive potentially on Mighty Silverfist

My character missing 2 passive from 12 (act1-3) despite I double check all possible quest and boss are re-killed. Mighty Silverfist icon is light up but do not drop passive book.

same problem here, i kill him alot time but nothing drop

I actually found the drop in titan sack as I forgot I have that after like 2-3 days
|
