omen of amelioration not working

just died to nothing i could see tbh, was walking in a lvl 81 ravine map (irradiated + corruption) in an empty space, suddendly got 1 shot from....i dunno...a bomb on the ground from nowhere?..... anyway, my omen is still in my inventory and just lost 10% xp at lvl 93.... -.-
fix this ty
