Ultimatum Lost

When I used the task utimatum from the dragon lizard for the test in 3rd Chapter and enter the Chaotic Test, the game crushed while im in the battle. And when I get back the utimatum disappeared (it is supposed to be on the place where you put in to enter the Chaotic test) and i cannot enter the Chaotic chapter again.
I cannot re-obtain by defeating the dragon lizard boss, can anyone help me with this?

Many people said if you dead from the test or won it the utimatum should remain in the "Temple Offering" statue but I checked there isn't anything there.
