Poor world interconnectivity in the campaign
There's no real sense of progression between Acts. Cutscenes and dialogue aren't really selling the transition from one place to another.
The same can be said of stage transitions too, a lot of stages end where a hole in the wall or a loop back would have been perfect. Teleporting out of dead ends doesn't feel good, a cinematic doorway or some clever loop back into previous territory would make things a lot more satisfying.
