Azak Pog Quest Spirit Bug

Killed the Azag Bog Witch in Act3 Cruel and ''picked up'' the Item.
The problem is my inventary was full and the item that gives me the +40 Spirit disappeared.
I made a new Instance and killed her again but she doesnt drop the item anymore.

The symbol on the map ( Skull with the + ) is still on my map even tho i returend the other quest item that drops from her to the quest dude.

Besides the Bug that needs to get fixed, is there a way for me to get my 40 spirit or is my char doomed now?

I'm still trying to figure this one out. One of my two normal spirit quests didn't give me the reward.

What did you end up doing? Please, help me get the last 30 spirit I need for my build!

For the next person who sees this - I was able to eventually get all of my spirit quests completed. I don't think there is a bug, but the UI can make it challenging to understand which quests/bosses you need to do.

Look at the in-game map legend- you're looking for the skull icon with a + next to it. Bosses you've killed have a 'faded' icon, bosses you haven't are solid looking.

For developers: When the skull drops next to The King in the Mist, it is super-not obvious that it's a separate clickable item from the ritual altar, ESPECIALLY on console! Make the skull drop farther away from the altar and I bet half these bug reports go away :)
