Ascent to power quest not clearing ACT2 Cruel.

I've done the first 2 ascenancies, on act 2 cruel, was given the quest again, cleared traitors passage and killed balbala again and the quest wont clear.

Also on a side note how do you get the 3rd and 4th ascendancies? / what level are they aimed for?
