What does "tier" on gear do?

Cant figure out what it does.

My guess it can't roll below the tier listed. So if you get a helm with Tier 4 and roll # to maximum life it can't roll below tier 4.

It means that a part of the "bottom range" of the affixes spread is removed. Instead of r0lling anywhere between 20-200 health (average 110), it can roll between 110-200 health instead (average 155).
