Melee Accuracy Question

As I'm progressing to higher level content as a Warrior I'm noticing that my attacks are missing more frequently, where recently it's become quite noticeable.

When I read the accuracy description it states there is no penalty to accuracy when you're within ~2m of the target. I'm going off the assumption 2m is within toe-to-toe distance with an opponent.

What am I missing here? I'm missing on average 20% of the time at level 22.
