augury doors stayed closed

I ran around the entire perimeter 3 times, cleared every monster, turned every lever I saw when I encountered them and the monsters in the middle were stuck behind locked doors. People told me there should have been a red line of sorts powering the doors but I saw nothing.

It happened again: I think what's causing this bug is being attacked while trying to use the lever. The animation completes, there is no option to redo the lever and the doors/lights never operate.
