Cannot reach quest item even after restarting instance

I died when the boss died and now I can't get down to the arena to get the quest item. I've tried restarting the instance as well. The quest item is "The Hammer of Kamasa."

+1 - I had that happen too, but you just have to wait some minutes for the instance to reset and you can run again and kill the boss, even though your quest status says to pick up the hammer. If your instance is reset, boss will live and be killable again.
